non in-patient care including:

5. Rehabilitation and chronic care etc

Both office and enterprise editions support scalability across organisations of differing size and service offerings.

Intrahealth has invested to build a platform for community
based care as:

• Few companies worldwide are investing in community care technology
• Inpatient systems do not fit well with the diversity of services community care requires
• HCC supports inclusive participation:

  – Patient (“Client”)
  – Care givers and Whanau
  – Research
• HCC provides an opportunity to approach the Personal Health Record as a collaboration between community health and consumers

Community care is a diverse set of delivery components

• General Practitioners, Homecare, District nursing, Specialists, Health promotion, Health protection, Non-inpatient hospital services, Rehabilitation
• The GP is a one of many, but a key component in care delivery

The delivery components are assembled in different ways in different markets – the Provider Network is the label we assign to this.

Community based services are highly diverse

• Many different services (mental health, sexual health, disease state management, health promotion, health protection, general practice, etc)
• Many different delivery mechanisms
  – Organisation structures: Corporate, trusts, collaboratives, co-ops, etc
  – Technology: PC, Internet, PDAs, etc

• Flexibility to allow technical users to easily extend and develop

• HCC has been implemented at 2 of New Zealand’s largest District Health Boards
• We have no inpatient solution customers to dilute our focus
• Approximately 200 community sites have been installed of various sizes
